Childermass winds up alone in a room with the fae - which actually ends up being pretty anticlimactic, over all. Instead of paying any attention to him what so ever, the fae just sets about meticulously filling in blanks on the form with the point of her claw, the letters forming as if with scratchy black ink. Obviously, a lot of the blanks are irrelevant to a fae and get somewhat nonsensical answers. Their home address is simply listed as "The Wylds."
They get everything filled in besides the actual signatures as they wait, looking fairly content, and take a moment to smell the tea as they wait. A suitable offering for their mercy, clearly.
