He really doesn't want to mess up the surgery or have Herbert hurt Ignatius in some way, so he is keeping his mouth shut and watching anxiously from the mirror like the love interest in any hospital drama when the patient of the day goes under the knife.

He is 100% not paying any attention to anybody trying to get his attention via reflection. Sorry Pap, your idea is good in theory but in practice, Strange's one track mind wins again.
